A cracked E90 or E92 headlight lens
If the lens on your E90 sedan or E92 coupe has cracked, from a rock or with no clear reason, the lights behind it may still work fine. The crack is what lets water and dust in.

E46 headlight lens: cracked, chipped or faded
A crack, a hole or a stone chip that went through the BMW E46 headlight lens lets moisture in, and the inside starts to fog.
A lens that is only faded can get a new lens too. Polishing it would take off the factory protective layer and the fade would come back, so the old lens comes off and a new one is sealed on.

E60, E39 and E70 headlights

Why a BMW lens is sealed again, not glued on
Home videos show the headlight baked in an oven to soften the glue. The risk is a warped housing, or a gap in the seal that fogs the headlight later.
In the repair, the old lens comes off, the sealing channel is cleaned and the new lens goes on with fresh sealant all the way around.
